Help with Care is building a neighbourhood coordination network in South West London. Joining means your existing residents can see you in their care plan, and residents in your area can identify you as a provider when they are looking for support.
Help with Care is open to any regulated or professionally qualified provider working with residents in South West London. This includes:
Eligibility is not limited to CQC registration. Physiotherapists, chiropractors, podiatrists, dentists, and many other independent practitioners hold their own professional body registration instead, such as the Health and Care Professions Council or the General Dental Council. GPs and individual physicians are covered by GMC registration. If you hold a recognised professional registration relevant to your field, you are likely eligible, whether or not that is CQC. If you are unsure, contact us and we will confirm.
Registration is straightforward and does not require you to change any of your existing systems.
Complete the registration form on this page. We ask for your organisation name, your professional registration or CQC number, your service area, and a contact name and email address.
A member of our team contacts you within five working days. We explain what being part of the network means in practice, answer your questions, and confirm your listing details.
Once confirmed, your organisation appears in the Help with Care provider directory. Residents and carers in your area can see that you are part of the network. Care coordinators can include you in care plans where you are the appropriate provider.
Before you can see resident information or take part in a referral, you complete a short onboarding session with our provider working group. This covers confidentiality and data handling, how referrals and shared care plans work in practice, and what residents have agreed to when they join Help with Care. This step is required, not optional, and applies to every provider before their first referral.
Once onboarding is complete, when a resident or their GP identifies you as the right provider for a care need, a referral arrives through the platform with the resident's consent and the relevant background already attached. You respond through the same channel.
As you work with a resident, you can add notes, mark appointments, and flag any changes to their situation. Every other provider in the care plan sees the same updated picture. If something needs clarifying, you can message the other providers in that resident's care pathway directly, or put the question to the wider network if needed, so care does not stall while you wait for an answer. At any handover, it is clear who is responsible for the next step. The resident's GP can stay informed for oversight, and can step in personally, or ask another professional, such as a social care worker or care provider, to do so.
Note: Steps 5 and 6 describe the intended operation of the platform as it is developed during the pilot. Initial coordination during the early pilot period is supported by our helpdesk team.
Your listing shows residents and carers in your area that you exist, that you accept referrals, and what conditions or services you cover. Many qualified providers are not visible to residents who would benefit from their services.
A referral from Help with Care comes with the resident's consent, their relevant history, and the details of the GP or care coordinator who raised it. You do not start from scratch.
When a referral is sent, both parties can see its status. There is no need to ring round to confirm receipt or chase an update. When you update the resident's record, the referring party sees the change immediately.
Your assessments, appointments, and notes are shared with every other provider involved in that resident's care.
Help with Care is designed to sit alongside the systems you already use, not to replace them. During the pilot, our helpdesk team handles the coordination work and keeps your administrative burden low.
Your listing works in two ways.
Residents, carers, and care coordinators can look up providers by service type and area, in the same way you might search any professional directory today.
Once you are registered and onboarded, you can refer a resident to another provider on the network directly, so care does not wait while everyone works out who else is involved. Where there is no existing pattern to follow, a resident recovering from a hip fracture for example, you can invite another registered provider to join that resident's care, in a similar way to sending a connection request. If they accept, you work together with a shared view of that resident's care, and further providers can be added the same way, one at a time, until everyone needed is part of the same picture.
During the pilot, we build this by looking up and adding qualified providers to the directory ourselves, as a free service. The referral and invitation features above describe how the platform is intended to work as it develops.

Residents who register with Help with Care agree to share relevant information with the providers involved in their care, and to let their care coordinator invite providers into their care plan where needed. This is what allows the coordination described on this page to happen in practice. Residents choose who is part of their care picture and can add or remove people at any time.
There is no charge to register during the pilot period. Our aim is to build a network of providers who are prepared to work in a more connected way. We will be open about any future commercial arrangements before they are introduced, and early partners will be consulted first.

The form takes approximately five minutes. We aim to make contact within five working days and complete your listing within ten.
We can remove your listing from view, so residents and providers can no longer select you as a new option, usually within two working days. If you are currently providing care to a resident through an existing care pathway, that needs to be discussed separately before your involvement ends, since a resident's active care cannot simply stop. We will contact you to agree the right next steps in that case.

We are setting up a working group of local providers to review how the network is performing and how the service can be improved. We welcome enquiries from providers who would like to join now, ahead of the pilot, so we can get to know each other and be ready to work well together from day one.
