Does any of this sound familiar?

Coming home from hospital
He came home with a bag of medication and a letter he couldn't properly explain to me afterwards. Nobody had thought to make sure I understood it too. His GP had not been told he was discharged. His physio appointment never arrived.
Sound familiar? It should not have to be this way.

With Help with Care
The discharge summary reaches every professional on the care record before he leaves the ward. Someone calls the following day to confirm every next step is in place.
Family spread across the country
My mum is in Kingston. The four of us live in different cities. We spend more time calling each other and chasing providers to work out what is happening than we do actually speaking to her.

Four adult siblings coordinating care for a parent from different locations
With Help with Care
All four siblings are named on the care record, each able to share what they learn and raise something with a provider without the other three needing to ring separately. Everyone sees the same outcome.
A child with complex needs
Between the paediatrician, the school nurse, the physio, and just working out what else he's even entitled to, I am the only one who knows what all of them have said. I keep a notebook. I am terrified of losing it.

Every specialist sees the same shared record. Notes, actions and upcoming appointments are visible to all of them - and to the parent holding everything together.
What happens when you register

Four steps to a care record
that works for you.

Registration takes a short phone call or a visit to our drop-in. From there, our team manages the coordination, though you'll still be asked for information or a decision only you can give.

1
We set up your record
A coordinator takes the details of your situation - your condition, your current providers, your family or carer - and builds your care record from that conversation. You check and approve it before any other provider sees it.
2
We contact the people involved
We reach out to your GP, your care provider, and whoever else is involved and invite them on to the shared record. You do not have to do this yourself.
3
Your carer is included
If you have a family member or unpaid carer, they are added to the record with the level of access you choose. They are a named part of the team from day one.
4
We stay in the background
You call us when you need us. We follow up on referrals that go quiet. We flag things to the right person. You are not navigating alone any more.

Register by phone, in person, or online, whichever suits you best. We handle the coordination and admin from there. Where something needs a decision only you can make, we will always come back to you.

What you can expect

Practical help that works without a screen.

Help with Care works well as an app or online, and just as well without one. It works for everyone - whether you are confident with technology or prefer to pick up the phone.

One shared record for everyone you choose
Your GP, your care provider, your community nurse, your family member. Everyone you invite sees the same up-to-date picture of your care.
Referrals you can see and track
When someone refers you on, you know it has arrived and what happens next. If it goes quiet for too long, we follow it up.
A helpdesk staffed by people who know your record
Call us and speak to someone who already knows your situation. You do not explain yourself from the beginning every time.
Your carer is a named part of the team
They receive the same updates you do. They are not an afterthought. They are on the record.
No device or login required
Phone, drop-in, or app - whichever works for you. Help with Care does not require technology to get started.

For unpaid carers

If you are caring for someone, we are here for you too.

There are 5.8 million unpaid carers in England. Most do not call themselves carers. Help with Care treats them as the essential part of the care team that they are - named on the record, kept informed, and entitled to support in their own right.

We are launching our pilot in South West London in 2026. Register below and we will be in touch when Help with Care is available in your neighbourhood. No commitment. No obligation.
